Plans for Aging in Place

The mission of Diamond Home Care is to maximize each person's strengths and abilities to perform the usual activities of daily living in a safe manner in order to remain in their own home. Our team works closely with senior community managers and residents to promote aging in place through individualized care programs.

A dedicated team is assigned to the senior community to promote continuity of care. Each person receives a complimentary RN visit to assist him/her and family in identifying the appropriate level of assistance needed. Trained caregivers are available to provide services up to 24 hours a day, 7 days a week including holidays.

"A la carte" services allow for individuals to select only specific services either ongoing or for a specific period of time. Graduated care plans provide multiple services in the most cost effective manner.

♦ The Ruby Plan - basic care plan designed for individuals requiring occasional assistance throughout the day

♦ The Emerald Plan – intermediate care plan for individuals with increasing morning and evening assistance required

♦ The Diamond Plan - advanced care plan that includes incontinence needs, laundry and supervision.

Diamond Home Care is also a Medicare/CHAP certified agency that promotes a safe transition to home while caring for someone recovering from illness or injury. The nurse along with our team of professionals including Physical Therapy, Occupational Therapy, Speech Therapy, Certified Nursing Assistants and Medical Social Workers work closely with the physician.
